I've cut a few holes in lids and found that for ease of use a 'U" shaped hole has worked best. It allows you to lift the lid without having to remove the immersion heater. I don't think it has to be a perfect tight fit. My experience has been that evaporations is not huge issue. I used a paper plate to make a template. Once I had a nice fit I traced it onto the lid. Now depending on what the lid is made of, making the cut-out can be tricky or supper easy.

Joule is 1.85 inches (47mm). Although hairyotter is right. I would make a U shaped hole so you can lift the lid without having to remove Joule.

Here is my set up...2" hole saw hole (don't rush the cut, go slow and gentle) finished with a saber saw for the cord. Super glue a steel washer in the bottom, remove the clip and put it away. I like this configuration because on long or high temp cooks you lose very little heat/water. I painted my washer red on the top (not the bottom) because I drink a lot and it is easier to know which end the Joule goes in....all my cambros are set up like this...
did the same with my insulated cooler...sorry no pics 'cause a friend has it this week...
